Contents Concept and design[] A monster with a concealed face was the concept behind Pyramid Head's design; Ito rejected his initial sketch (left), which resembled a masked human, and gave the creature a pyramid-shaped helmet (right).Ito wanted to create "a monster with a hidden face", but became unhappy with his designs, which resembled humans wearing masks. He then drew a monster with a pyramid-shaped helmet. According to Ito, the triangle's sharp right and acute angles suggest the possibility of pain. Of the creatures that appear in Silent Hill 2, only Pyramid Head features an "overtly masculine" appearance. He resembles a pale, muscular man covered with a white, blood-soaked robe reminiscent of a butcher's . He does not speak, but grunts and moans painfully. His most outstanding feature is his large red, triangular helmet. His weapons consist of the deadly and heavy Great Knife, which the player can find and use for the rest of the game, and later a spear. As a German enthusiast, Ito was inspired by the lower hull of the heavy tank when designing the edges of the character's helmet; other military vehicles from that region and era informed design elements as well.

According to Konami's Lost Memories, his appearance was a variation of the outfits of the executioners from the fictional history of the town. They wore red hoods and ceremonial robes to make themselves similar to Valtiel, a monster who appears in . Like Valtiel, Pyramid Head dresses in gloves and stitched cloth and pursues the game's main protagonist. According to Silent Hill 2's character designer Takayoshi Sato, he appears as a "distorted memory of the executioners" and of the town's past as a place of execution.

, the director of the film adaptation of , suggested that Pyramid Head "was one of the executioners in the original history of the town" and "there is not one particular or exclusive manifestation of him as an entity."

Appearances[] In video games[] In , after receiving a letter from his deceased wife, Mary, and arriving in the foggy town of Silent Hill to search for her, the game's protagonist and primary player character, James Sunderland, encounters Pyramid Head several times over the course of the game. He first appears from behind a gate, making no attempt to attack James. Later, in an apartment, James walks in on Pyramid Head sexually abusing and then killing two Mannequins—creatures made of two sets of feminine hips and legs. Terrified, James hides in a closet and watches while Pyramid Head stands directly outside of it before leaving. When James asks another character, Eddie, about the monster, Eddie denies knowing about Pyramid Head. Later, near a flooded stairway, James witnesses Pyramid Head engaged in another act of sexual torment with a monster, and Pyramid Head attempts to kill him, causing a boss fight. After a few minutes, sirens sound in the distance and Pyramid Head descends the stairway and disappears. James does not meet him again until in Brookhaven Hospital, where Pyramid Head knocks him through a safety railing and he falls, sustaining injuries. Pyramid Head does not pursue him or continue the attack. Later, he stalks James' companion Maria, who closely resembles his wife Mary, through a lengthy corridor. As James flees into an elevator, the doors shut before she can join him. He struggles to open the doors to save her, but Pyramid Head kills her. However, in the labyrinth beneath Toluca Prison, James finds her alive and unharmed in a locked cell. Before trying to seduce him, she reminisces about a trip that only he and Mary took to a hotel in Silent Hill. He leaves, promising to find a way to free her, and discovers that Pyramid Head walks a corridor nearby, now carrying a spear. Afterwards, James reaches Maria's side of the cell, but finds her dead. Pyramid Head makes his final appearance just before the final boss, where two Pyramid Heads take part in the encounter. They kill Maria yet again and, after pursuing James around the room for a while, both commit suicide.

Pyramid Head, under the new moniker "Bogeyman", makes a significant appearance in 2008 installment , but his role is limited to non-interactive scenes, appearing only twice to the game's main protagonist Alex Shepherd (once in the Grand Hotel in Silent Hill and, much later, in a church, where he executes Alex's father by splitting him in half). His last appearance is in a possible ending to the game: Alex wakes up in a wheelchair as two Pyramid Heads appear, each with part of a helmet, which they use to turn Alex into one of them. After this ending is played, the player obtains the Bogeyman's costume for Alex to wear. Mindful of Pyramid Head's role in Silent Hill 2, the developers chose to include him as "the embodiment of a myth [that] parents started to keep the children out of trouble" and "the accretion of the activities going on in the town of Shepherd’s Glen."

Pyramid Head appears as a boss in the 2007 first-person shooter , as well as , but with a composite design with the Butcher, and as a selectable character in the 2008 title , an installment of the series, another spin-off alongside Robbie the Rabbit, and in 2017's . The 2007 game also included a similar monster named "The Butcher", whom the protagonist occasionally encounters killing other monsters (unlike other versions of Pyramid Head, the Butcher can and must be fought and killed in combat) and a painting of Pyramid Head is seen in the burning house of Alessa. Pyramid Head appears in 2012 installment , playing more prominent role in main story and in "Surprise!" ending, along with various characters from the franchise. Pyramid Head's most recent appearance was in 2012's , where he is a monster that can spawn randomly to fight the player. Pyramid Head also appeared as a costume for the Playstation Home.

On a update released on June 29, 2017, Pyramid Head appears as a cameo as Pyramid Bomber, alongside from the series, and 's Vic Viper.

On May 26th, 2020, Pyramid Head was announced as the newest killer for the survival horror game .

Pyramid Head makes an appearance in the as "Red Pyramid", and is portrayed by Roberto Campanella. In the film, the psyches of the female characters shaped the character's physical appearance. Gans claimed that replicating the character's head exactly and having the actor move while wearing it proved to be impractical; he noted that, despite the name, Pyramid Head actually wore "a basin" instead of a triangle-shaped helmet. Red Pyramid's sword and helmet were constructed out of lightweight material painted to appear heavy. For the role, Campanella wore a "five-part "; it took two-and-a-half to three hours to get him into costume and make-up. His boots had a hidden 15-inch (38 cm) sole which made him just under 7 feet (2.1 m) tall. , who worked on the make-up effects and monsters, enjoyed the project of designing the character. According to him, Red Pyramid serves as a symbol of the town's darkness and harbinger of its changed character. Gans considered the monsters of the film "a mockery of human beings", and commented: "The real monsters are the people, the cultists who tortured Alessa. When I approached the film, I knew that it was impossible to represent the monsters as simply beasts that jump on you."

In the 2012 film , the Pyramid Head, once more portrayed by Roberto Campanella, is both a monster and a guardian. More than once, it saves Sharon from the forces of Silent Hill. This is explained by its loyalty to Alessa: since Heather is the good half of Alessa's soul, it sees them as the same being and is thus bound to protect both from harm. Pyramid Head is also seen controlling the carousel where Heather and Alessa face off against each other, then it kills the cult leader Claudia Wolf after the amulet shows her inner, monstrous nature.

He also makes a in the 2008 Sinner's Reward, published by . The writer, Tom Waltz, later said he regretted the cameo, which only functioned as . To him, Pyramid Head is a psychological construct created for James; however, he stated that: "At the same time, I don't think that ruined the comic. Some people really liked it. To some people, Pyramid Head should be in all the stories because they do like him." The Silent Hill series uses symbolism and psychology; the town of Silent Hill draws upon the psyche of its visitors, ultimately creating an "," a twisting of reality that manifests delusions and elements of their , varying from character to character. Specifically, in the case of Silent Hill 2's primary player character James Sunderland, the version of the town he explores is influenced by him. Many of the monsters that roam the town symbolize his guilt, wish for punishment, for his wife's three-year-long illness, and cease to exist after James comes to terms with the fact that he killed his wife Mary, partially to end her suffering and partially out of resentment and frustration. James knew she had a terminal illness, which has been speculated to be cancer, and he often read medical textbooks, searching for something to help her. During her last days alive, she became physically repulsive as a result of the illness and treated James abusively, ordering him to leave one moment and begging him to comfort her the next. The knowledge of her terminal illness caused her to become angry and to hurt her loved ones, particularly James, and it pained him to visit her in the hospital. Pyramid Head functions as an executioner of Maria, a delusion of James' who strongly resembles Mary. Through Maria's repeated deaths, Pyramid Head reminds James of Mary's death and causes him to experience guilt and suffering. His appearance as an executioner stems from a picture that James saw while visiting the town three years ago with Mary.

Reviewers have suggested various interpretations. According to Christina González of , Pyramid Head acts as "judgement personified, a dark butcher," and "James' masochistic delusion" which punishes him for Mary's death. Ken Gagne of suggested that the monster "represents James' anger and guilt." A critic for , Jesse Schedeen, considered Pyramid Head's role throughout the Silent Hill series to be "a manifestation of a person's guilt", commenting: "Pyramid Head is just there to help you serve your penance in the most painful and grotesque way possible."

Reception[] Critical reaction to Pyramid Head has been favorable because of his distinctive appearance and role as an element of James' . Critics cite him as an iconic villain of the Silent Hill series, a favorite among fans, and part of the appeal of Silent Hill 2. compared Pyramid Head's appearance to , the main antagonist of series of , and found him the most terrifying monster in Silent Hill 2. named Pyramid Head as one of the most terrifying villains in computer and video games. felt that the scene in which he rapes the two other monsters was unsettling, since the subject of rape is not often tackled in video games, and disliked the final battle with him because of how anti-climactic it was, in comparison with his role throughout the rest of the game.

ranked Pyramid Head first on their 2008 list of the scariest video game monsters. GamesRadar listed him as one of the 25 best new characters of the decade and ranked him second on its 2009 list of the scariest video game villains, calling him the "most horrifying character ever to have a cult following". In a 2009 retrospective on the genre, noted that Silent Hill 2 incorporated "a 'stalker' element similar to and ," and wrote: "To this day, Pyramid Head is remembered as one of gaming's most frightening villains." ranked him as the first on their 2009 list of the scariest monster "to infest a platform", also noting his "prolific reputation as one of video game’s most fearsome monsters." In 2010, Pyramid Head as named as the scariest character in video game history by , who added that "being chased by him in Silent Hill 2 ranks with all of the scariest moments in video gaming history." ranked him as number one on their 2011 list of the scariest characters in video games, commenting he is "too mainstream to be all that scary, except he's still really super scary." In 2012, included him on their lists of the biggest freaks and scariest characters in the games, commenting: "Ol’ Metal Top is easily the best thing about Silent Hill – and maybe even survival horror in general – completely encapsulating that intoxicating dread of knowing that it’s not if you’ll fight him, but when." That same year, included him on their list of ten scariest game characters ever; on the list of the "coolest" video game villains by , Pyramid Head placed as 15th. GamesRadar too praised Pyramid Head's role as an antagonist, putting him in their 2013 list of the best villains in video game history at number 18.

His appearances outside Silent Hill 2 have received mixed critical reaction. Critics generally agreed that Pyramid Head's appearance in Homecoming struck them as , though Chris Hudak of called it "damned effectively-employed." Masahiro Ito commented that Pyramid Head was designed specifically for James' story in Silent Hill 2, and that, if he were to make another Silent Hill game, he would either not use Pyramid Head again or kill him off entirely in the opening scene. Film critics commented on his role in the film adaption, with several finding him disturbing. DVD Reviews praised Campanella's portrayal of Pyramid Head and another monster, writing: "These are without a doubt some of the most striking bogeymen that I have seen on screen in a long time." His appearance in received mixed critical reaction. GameDaily disliked it, finding it awkward that a character like Pyramid Head was competing in sporting events with characters like and , while The Escapist called it enjoyable and "hilarious".

Because the monsters of are manifestations of protagonist 's subconscious and not pre-existing creatures, designer wanted them to have a symbolic meaning applying to James' character. When designing Pyramid Head, he wanted a monster with a hidden face so that it would appear less human and more disturbing. He scrapped his original design for Pyramid Head, which to him looked too much like a regular human in a mask. He redesigned it in a large pyramid-head shaped helmet and butcher's smock to get the inhuman quality he wanted. As a tank enthusiast, Ito took inspiration from the lower hull of the "King Tiger" German WWII tank when designing the edges of Pyramid Head's helmet, as well as avante-garde and other vehicles from the period. He also took influence from oil/acrylic painting he created in 1995 while in art school depicting a humanoid pyramid-shaped object as part of his Strange Head series. According to Ito, some of the members and his boss were hesitant about including the design at first for its unconventional nature, but this did not impact development.

After doing art direction, environment designs, key frame animations, and all of the creature design in Silent Hill 2, Masahiro Ito returned as a creature designer and art director for . The extensive work on the series made him exhausted. After was released, he was asked to work on another Silent Hill game. However, because they wanted him to include Pyramid Head under a different name and look, and because he had lost a lot of motivation to keep working on horror games, he turned down the project. A similar monster called the was ultimately featured in the prequel . A for the game had the protagonist going insane and becoming Pyramid Head. Pyramid Head was featured in the for a new Silent Hill game by the , which was rejected by .

Pyramid Head played a role in the 2006 and under the name Red Pyramid, played by . Despite being an element of James Sunderland's psyche, reused the character with his film design in the video game . When full development started, producer protested the inclusion of Pyramid Head to the point that he was removed from the game's production and placed on other Konami games until another producer left and he was brought back on board. Ito criticized the inclusion and musculature of Pyramid Head in Homecoming , stating that "I really disliked muscular enemies being made appearance in horror titles without a reason. In my opinion, enemies should have a reason for being in it. And PH in James' story had a reason to have to be not muscular." Lead designer defended the game by assuring fans that even though James Sunderland is absent, the monster serves a specific purpose. He referred to Pyramid Head as "integral" to Silent Hill lore and that the decision to insert him was not taken lightly, also acknowledging that he represented James' desire for punishment in Silent Hill 2. Hulett attempted to reconcile the continuity error by inserting the into the game that dubbed this new incarnation "the Bogeyman, the Shadowed One", giving him a new name and reinventing his purpose for existing. He felt that if Pyramid Head had to be included, he needed a central role to the story rather than just a cameo.

Ito was displeased with Pyramid Head's inclusion in the later games, believing that he strayed too far from his original purpose. He also felt that it would be difficult to convince Konami to make a new game without him, because many of the producers believe that the brand recognition of the character makes too much money. If Pyramid Head was to be used again, Ito wanted him to have an "impressive new direction." When he became involved in a , he designed brand new iterations of and Pyramid Head that were distinct from what was seen in Silent Hill 2 and . The intention was for the more angelic Valtiel to kill Pyramid Head in the opening scene of the game. Concept art for the opening depicts a baby carriage on a bridge in the Otherworld. The game was cancelled, but Masahiro Ito stated in 2017 that he would only be willing to work on a new Silent Hill project if Pyramid Head is not present or if he can be killed off in the opening.

In the religion of Silent Hill, Valtiel exists as one who is close to , and Pyramid Head takes the shape of Valtiel's reason for existence. There is a supernatural power that surrounds that has the ability to manifest elements of the unconscious mind. In the early history of the town, Native Americans used this to seemingly communicate with both nature and their dead relatives. Their land was eventually stolen from them by European settlers. Because of the town's power, a mysterious that took control over much of its infrastructure. One of the deities they worship is , who serves as an angel of rebirth and a being close to God who has appeared in many forms throughout the town's history. The cult began performing ritualistic executions, wearing red hoods and an executioner's outfit modeled after him.

Right before contracted a lethal disease, three years before the events of Silent Hill 2, she and her husband visited the town of Silent Hill on vacation. One of the locations they visited was the , which contained and artifacts relating to the executioners of the town's past. One such painting in particular, "Misty Days, Remains of the Judgement" (a stylized portrayal of an executioner standing among caged corpses) stuck in James' mind.

In the final stages of her disease, Mary fell into a state of agony and became verbally abusive towards James. It was during this that James smothered her with a pillow to relieve her of her pain and because of his growing resentment towards her. He instantly fell into a state of regret and denial, later receiving a . With the memory of killing her having been repressed, he traveled to Silent Hill to find her. But as he arrived, he found himself in a where the darkest elements of his subconscious were manifested into twisted monsters and scenerios.

In his search for Mary, he made his way to the where he encountered Pyramid Head, a monster similar to the painting he had observed in the Historical Society years earlier. He hid in a closet as the creature abused and slaughtered two monsters known as . Having evaded Pyramid Head, James encountered it again in the , where it proceeded to attack him. After outrunning the assault, James watched as Pyramid Head fled the encounter. At the apartments, James interrupted a suicidal , taking her knife before she has a chance to use it, and met , an often bullied man who was self-conscious about his weight.

After going through the buildings, James followed the letter's instructions and made his way to . While Mary was nowhere to be found, he met a woman strongly resembling his wife named , who begged him to escort her through the dangerous town. After searching through the area, they came to where the town cycled to an even deeper part of the nightmare known as the . When they entered the hospital basement, Pyramid Head chased behind them as they sprinted to an elevator. James managed to make it to safety, but Maria was slaughtered for moving too slowly. The elevator took him to the first floor of the hospital, where a distraught James found a note instructing him to go to the Historical Society, which he had visited years prior. Upon arriving, he descended down a long stairway and kept jumping down mysterious dark holes until arriving in a in an .

Finding his way through the Labyrinth, James found Maria mysteriously alive and alternating between Mary's personality and her own. She tells James of the time he and Mary spent at the , and he leaves to navigate his way through the Labyrinth. Pyramid Head can be found in this area, but he does not pursue James unless directly approached. When James finds Maria again, she has once again been murdered. Making his way out of the Labyrinth, he comes face-to-face with Eddie, who has been driven insane from his experiences in the town. Eddie tries to murder him, and James is forced to kill him in self-defense. Shaken by the experience, James questions whether Mary really died three years prior as he remembers.

James makes his way to the Lakeview Hotel, where he finds a video tape recording of him killing Mary and suddenly recalls his past actions. He encounters Pyramid Head again, this time with a second Pyramid Head by his side. James realizes that his subconscious created them because of his guilty desire for punishment and decides that he no longer needs them. After a lengthy conflict, the two Pyramid Heads kill themselves, having fulfilled their purpose.

Although unseen, the Bogeyman makes himself known to in the game's introductory level, . His first action is the murder of the doctor who wheels Alex into the operation theater. As the player progresses through the hospital, a grinding noise can be heard periodically. Presumably, this is the Bogeyman dragging his knife across the floor. As the first level ends, Alex enters an elevator, following his . As it comes to a stop, the Bogeyman's knife plunges through the still-closed elevator door toward Alex. The scene then transitions to a truck cab and an awakened Alex, revealing that he was dreaming.

The Bogeyman only appears physically twice to Alex. He is first encountered in a cutscene in the in Silent Hill; seen dragging his knife down a hallway and being trailed by (similar to the film), he turns to face Alex, who has hidden himself behind a pile of rubble. The Bogeyman turns away and continues walking down the hall. Much later, within the , he appears a final time and executes Adam, cutting him in half lengthwise right before Alex's eyes.

In the 2006-2012 film universe, Pyramid Head was adapted as the monster Red Pyramid played by . In , he arrives to attack and the inhabitants of when the town transitions into the Otherworld. His arrival is heralded by swarms of . After a few failed attempts to kill Rose and , Red Pyramid pursues the protagonists to a where they manage to escape. A member of , falls behind and Red Pyramid brutally murders her.

The sequel reveals Red Pyramid to be the protector of , who had now been reincarnated as .

Appearance and symbolism Pyramid Head is a figure of James Sunderland's guilt and inner torment, manifesting from the guilty part of his mind that desires punishment. He is described as a "distorted memory of the executioners" by , who also explains that Silent Hill used to be a town of executions. Most of the people living there were either executioners or family to an executioner. A reflection of James, the character is an overtly masculine humanoid who takes an appearance similar to the executioners of the town's cult, with a red rusted pyramid-shaped helmet over his head, donning butcher's wordrobe, leather boots, and white executioner's gloves. There is a mark on the back of his garment where the cloth is stitched together, which is a similarity he shares with Valtiel. Coming out of the back-end of the helmet is a mysterious bubble-shaped protrusion, the purpose of which is kept a secret by creature designer Masahiro Ito. Some action figures have depicted it as inhuman flesh lining the bottom of the helmet. However, this design is not accurate.

The front of the helmet is an iron plate. Underneath it is tongue-like flesh that Pyramid Head uses when attacking James and other monsters. The bottom left corner of the plate will peel up, allowing for the tongue to slither out of the helmet. Ito used this to subtly convey that the monsters are James' delusion, since it is impossible for iron to bend. Everybody is thinking and concerning about sex and death. Everything. If we want to scare, shake, or touch the users or spectators, then we have to think about sex and death deeply. To make like a death scene, somebody died or monsters died, we tried to mix erotic essence. This is kind of a visual and a core concept." Pyramid Head's constant murder of a reincarnating is also his attempt to wake James up from his delusion and force him to remember his past actions.

In Silent Hill: Homecoming, Double Helix Games wanted to explore the nature of Pyramid Head. The Bogeymen are the embodiment of a myth that parents used to control their children. Jason Allen also referred to the Bogeymen as the "accretion of the activities going on in the town of ." The myth is perpetuated by a nursery rhyme, warning children that if they do not obey their parents, the Bogeyman will inflict punishment onto them. Throughout the game, the Bogeyman never attacks Alex Shepherd, targeting his sinful parents instead.

It is unknown what Pyramid Head looks like underneath the helmet. Masahiro Ito stated that he never designed Pyramid Head's face, but had an idea of "a binded someone's head with many frames" at the time of creating him. However, he has refused to reveal what the character truly looks like underneath the helmet, believing that it is more effective to leave it to the player's imagination. He added that he knows what the character looks like underneath the helmet, even though he chooses not to reveal it. The guidebook speculates that Pyramid Head's face could be similar to Valtiel's. An ending in depicts two Bogeymen transforming Alex Shepherd into one of them, indicating that the "Bogeyman" iterations of Pyramid Head could be those who were wronged in the town's history. However, Ito has stated that the Pyramid Heads are not men, but creatures.

In the film universe, Red Pyramid wears a basin with sharper corners and has a significantly stronger, more superhuman musculature than James' Pyramid Head. He wears an apron made of human skin with no top covering his upper torso. , the director of the first Silent Hill film, had tried to give him a more game-accurate appearance, but the shape of the helmet was impractical. They also wanted to update his appearance so that he appeared more powerful, rather than the grotesque humanoid with bad posture seen in Silent Hill 2. This was an homage to . The psyches of the female characters influenced his shape, dress, and physicality. Gans consulted composer and Konami before moving forward with Red Pyramid's final design. Rather than merely being influenced by the executioners of the town's past, the film iteration is stated to have literally been one of the executioners by Gans. He believes that "there is not one particular or exclusive manifestation of [Red Pyramid] as an entity."

Revelations director said of the character, "one of the big things is that people compaling that Red Pyramid belongs in game number two. But he was in the first film, so I couldn’t just ignore this amazing character in the sequel. He’s our , our posterboy. But he needed to make sense within the story." Whereas the first film was about a parent searching for a daughter, the second film is about the daughter searching for her father. This impacted Red Pyramid's role in the film, who is seen as a symbol of the power of masculinity and how it can simultaneously protect and oppress. As Shaun Jooste explains, "It is not the same PH that is in any of the games, but rather a new antagonist that is within that helmet. I might be able to tell the story without PH, but not without the person portraying him inside that helmet. I had read somewhere that those who serve Valtiel wear objects to represent the way his face is hidden too. Whether or not that is actually what is accepted, that stuck with me and I loved it. Because the monsters of are manifestations of protagonist 's subconscious and not pre-existing creatures, designer wanted them to have a symbolic meaning applying to James' character. When designing Pyramid Head, he wanted a monster with a hidden face so that it would appear less human and more disturbing. He scrapped his original design for Pyramid Head, which to him looked too much like a regular human in a mask. He redesigned it in a large pyramid-head shaped helmet and butcher's smock to get the inhuman quality he wanted. As a tank enthusiast, Ito took inspiration from the lower hull of the "King Tiger" German WWII tank when designing the edges of Pyramid Head's helmet, as well as avante-garde and other vehicles from the period. He also took influence from oil/acrylic painting he created in 1995 while in art school depicting a humanoid pyramid-shaped object as part of his Strange Head series. According to Ito, some of the members and his boss were hesitant about including the design at first for its unconventional nature, but this did not impact development.

After doing art direction, environment designs, key frame animations, and all of the creature design in Silent Hill 2, Masahiro Ito returned as a creature designer and art director for . The extensive work on the series made him exhausted. After was released, he was asked to work on another Silent Hill game. However, because they wanted him to include Pyramid Head under a different name and look, and because he had lost a lot of motivation to keep working on horror games, he turned down the project. A similar monster called the was ultimately featured in the prequel . A for the game had the protagonist going insane and becoming Pyramid Head. Pyramid Head was featured in the for a new Silent Hill game by the , which was rejected by .

Pyramid Head played a role in the 2006 and under the name Red Pyramid, played by . Despite being an element of James Sunderland's psyche, reused the character with his film design in the video game . When full development started, producer protested the inclusion of Pyramid Head to the point that he was removed from the game's production and placed on other Konami games until another producer left and he was brought back on board. Ito criticized the inclusion and musculature of Pyramid Head in Homecoming , stating that "I really disliked muscular enemies being made appearance in horror titles without a reason. In my opinion, enemies should have a reason for being in it. And PH in James' story had a reason to have to be not muscular." Lead designer defended the game by assuring fans that even though James Sunderland is absent, the monster serves a specific purpose. He referred to Pyramid Head as "integral" to Silent Hill lore and that the decision to insert him was not taken lightly, also acknowledging that he represented James' desire for punishment in Silent Hill 2. Ito was displeased with Pyramid Head's inclusion in the later games, believing that he strayed too far from his original purpose. He also felt that it would be difficult to convince Konami to make a new game without him, because many of the producers believe that the brand recognition of the character makes too much money. If Pyramid Head was to be used again, Ito wanted him to have an "impressive new direction." When he became involved in a , he designed brand new iterations of and Pyramid Head that were distinct from what was seen in Silent Hill 2 and . The intention was for the more angelic Valtiel to kill Pyramid Head in the opening scene of the game. Concept art for the opening depicts a baby carriage on a bridge in the Otherworld. The game was cancelled, but Masahiro Ito stated in 2017 that he would only be willing to work on a new Silent Hill project if Pyramid Head is not present or if he can be killed off in the opening.

In the early history of the town, Native Americans used this to seemingly communicate with both nature and their dead relatives. Their land was eventually stolen from them by European settlers. Because of the town's power, a mysterious that took control over much of its infrastructure. One of the deities they worship is , who serves as an angel of rebirth and a being close to God who has appeared in many forms throughout the town's history. The cult began performing ritualistic executions, wearing red hoods and an executioner's outfit modeled after him.

Right before contracted a lethal disease, three years before the events of Silent Hill 2, she and her husband visited the town of Silent Hill on vacation. One of the locations they visited was the , which contained and artifacts relating to the executioners of the town's past. One such painting in particular, "Misty Days, Remains of the Judgement" (a stylized portrayal of an executioner standing among caged corpses) stuck in James' mind.

In the final stages of her disease, Mary fell into a state of agony and became verbally abusive towards James. It was during this that James smothered her with a pillow to relieve her of her pain and because of his growing resentment towards her. He instantly fell into a state of regret and denial, later receiving a . With the memory of killing her having been repressed, he traveled to Silent Hill to find her. But as he arrived, he found himself in a where the darkest elements of his subconscious were manifested into twisted monsters and scenerios.

In his search for Mary, he made his way to the where he encountered Pyramid Head, a monster similar to the painting he had observed in the Historical Society years earlier. He hid in a closet as the creature abused and slaughtered two monsters known as . Having evaded Pyramid Head, James encountered it again in the , where it proceeded to attack him. When they entered the hospital basement, Pyramid Head chased behind them as they sprinted to an elevator. James managed to make it to safety, but Maria was slaughtered for moving too slowly. The elevator took him to the first floor of the hospital, where a distraught James found a note instructing him to go to the Historical Society, which he had visited years prior. Upon arriving, he descended down a long stairway and kept jumping down mysterious dark holes until arriving in a in an .

Finding his way through the Labyrinth, James found Maria mysteriously alive and alternating between Mary's personality and her own. She tells James of the time he and Mary spent at the , and he leaves to navigate his way through the Labyrinth. Pyramid Head can be found in this area, but he does not pursue James unless directly approached. When James finds Maria again, she has once again been murdered. Making his way out of the Labyrinth, he comes face-to-face with Eddie, who has been driven insane from his experiences in the town. Eddie tries to murder him, and James is forced to kill him in self-defense. Shaken by the experience, James questions whether Mary really died three years prior as he remembers.

James makes his way to the Lakeview Hotel, where he finds a video tape recording of him killing Mary and suddenly recalls his past actions. He encounters Pyramid Head again, this time with a second Pyramid Head by his side. James realizes that his subconscious created them because of his guilty desire for punishment and decides that he no longer needs them. After a lengthy conflict, the two Pyramid Heads kill themselves, having fulfilled their purpose.

Although unseen, the Bogeyman makes himself known to in the game's introductory level, . His first action is the murder of the doctor who wheels Alex into the operation theater. As the player progresses through the hospital, a grinding noise can be heard periodically. Presumably, this is the Bogeyman dragging his knife across the floor. As the first level ends, Alex enters an elevator, following his . As it comes to a stop, the Bogeyman's knife plunges through the still-closed elevator door toward Alex. The scene then transitions to a truck cab and an awakened Alex, revealing that he was dreaming.

The Bogeyman only appears physically twice to Alex. As Sato explains, "Psychological horror has to shake human's heart deeply. Shaking people's heart deeply means uncover people's core emotion and their core motivation for life. Everybody is thinking and concerning about sex and death. Everything. If we want to scare, shake, or touch the users or spectators, then we have to think about sex and death deeply. Masahiro Ito stated that he never designed Pyramid Head's face, but had an idea of "a binded someone's head with many frames" at the time of creating him. However, he has refused to reveal what the character truly looks like underneath the helmet, believing that it is more effective to leave it to the player's imagination. He added that he knows what the character looks like underneath the helmet, even though he chooses not to reveal it. The guidebook speculates that Pyramid Head's face could be similar to Valtiel's. An ending in depicts two Bogeymen transforming Alex Shepherd into one of them, indicating that the "Bogeyman" iterations of Pyramid Head could be those who were wronged in the town's history. In the film universe, Red Pyramid wears a basin with sharper corners and has a significantly stronger, more superhuman musculature than James' Pyramid Head. He wears an apron made of human skin with no top covering his upper torso. , the director of the first Silent Hill film, had tried to give him a more game-accurate appearance, but the shape of the helmet was impractical. They also wanted to update his appearance so that he appeared more powerful, rather than the grotesque humanoid with bad posture seen in Silent Hill 2. This was an homage to . The psyches of the female characters influenced his shape, dress, and physicality. Gans consulted composer and Konami before moving forward with Red Pyramid's final design. Rather than merely being influenced by the executioners of the town's past, the film iteration is stated to have literally been one of the executioners by Gans. He believes that "there is not one particular or exclusive manifestation of [Red Pyramid] as an entity."

Revelations director said of the character, "one of the big things is that people compaling that Red Pyramid belongs in game number two. But he was in the first film, so I couldn’t just ignore this amazing character in the sequel. He’s our , our posterboy. But he needed to make sense within the story." Whereas the first film was about a parent searching for a daughter, the second film is about the daughter searching for her father. This impacted Red Pyramid's role in the film, who is seen as a symbol of the power of masculinity and how it can simultaneously protect and oppress. The second film expands Red Pyramid's role to be not just that of a former executioner, but also as a monster created as a guardian to Alessa and her reincarnation, Heather.

In Silent Hill: Betrayal, Pyramid Head is once again reinterpreted, this time as a servant of Valtiel, a guardian, and a bringer of righteous vengeance and justice. As Shaun Jooste explains, "It is not the same PH that is in any of the games, but rather a new antagonist that is within that helmet. I might be able to tell the story without PH, but not without the person portraying him inside that helmet. As such, PH becomes an essential plot character rather than an obstacle [the protagonist] needs to bypass. I had read somewhere that those who serve Valtiel wear objects to represent the way his face is hidden too. Whether or not that is actually what is accepted, that stuck with me and I loved it. And since my series takes a much closer look at Valtiel as my favourite deity of the series, I wanted the same guardians in the novels." , who wrote some of the IDW Silent Hill comics, later regretted including Pyramid Head in . He was not aware that the monsters manifest from the subconscious of the characters at the time, and included Pyramid Head because of his design. He said, "In hindsight, I know better."
